Job Description

Performs evaluation tasks, including vital signs and medication review

Monitors patient recovery and compile reports for the attending Physician

Assess the patient’s condition during every visit and chart every observation

Administer medication as prescribed by the the physician
Provide palliative care as needed to keep the patient comfortable

Educate patients and their families on proper home healthcare strategies and procedures
